Our transportation program offers rides that make it easier for older adults to access care, run errands, and stay active in the community they love.

Transportation

This service is available to people 60 and older who are socially isolated because of a lack of transportation. Due to limited funds, transportation services are offered to qualified adults primarily in the Manchester area. Additional transportation needs are met as funding and service hours permit.

Nutrition Screening
The following are true/false questions to complete the Nutritional Risk Screening. Please circle true or false based  on how the statement applies to you, and add up the corresponding points to identify your nutrition risk score. If  your total score is 6 or higher, we encourage you to follow up with your healthcare team for nutrition support
including education and nutrition counseling, if appropriate.
